Melissa’s scream brought two nurses rushing into the room.

“Please lower your voice,” one of them said. “The baby needs to be examined.”

But Melissa couldn’t move.

She was staring at the newborn’s back.

So was Ryan.

I held the baby closer to my chest.

“What am I supposed to be looking at?”

The nurse gently adjusted the blanket.

And then I saw it.

A small, dark-red birthmark sat just below the baby’s left shoulder.

It wasn’t large.

It wasn’t dangerous.

But around it was something that made my stomach twist.

A thin, unusual pattern that looked almost like a crescent surrounded by three tiny dots.

Ryan stepped backward.

“No.”

I looked at him.

“Ryan, what is that?”

He shook his head.

“I knew this could happen.”

Melissa turned toward him.

“Knew what could happen?”

Ryan didn’t answer.

The doctor examined the baby carefully.

After several minutes, she looked confused.

“It appears to be a congenital birthmark. I don’t see anything medically alarming.”

Melissa stared at her husband.

“Then why did you refuse our daughter?”

Ryan finally looked at her.

His eyes were filled with tears.

“Because I’ve seen that mark before.”

My heart stopped.

“Where?”

He looked directly at me.

“On Dad.”

The room went completely silent.

Our father had died twelve years earlier.

I remembered the small birthmark on his shoulder, but I had never paid much attention to it.

Ryan continued.

“After Dad died, I found an old photograph of him when he was young. The mark was there.”

Melissa shook her head.

“That doesn’t mean anything.”

“I thought so too.”

Ryan pulled his phone from his pocket.

His hands were shaking as he opened an old photograph.

He showed it to us.

It was our father standing beside a woman I had never seen before.

And on his shoulder was the exact same mark.

But that wasn’t what made me go cold.

The woman beside him was holding a baby.

A newborn girl.

I stared at the photograph.

“Ryan…”

He swallowed.

“That picture was taken before you and I were born.”

Melissa’s face went pale.

“Who is that woman?”

Ryan looked at me.

“I don’t know.”

Then he said something that made my knees weaken.

“But Dad spent years paying her money.”

I looked down at the baby in my arms.

“Why?”

Ryan didn’t answer.

Instead, he reached into his jacket and pulled out an old envelope.

“I found this six months ago when I was cleaning out Dad’s storage unit.”

He handed it to me.

There was one name written across the front.

MELISSA.

My eyes widened.

“This is addressed to your wife.”

“I never opened it.”

Melissa grabbed the envelope.

Her fingers trembled as she tore it open.

Inside was a single handwritten page.

She read the first sentence.

Then stopped breathing.

“What does it say?” I whispered.

Melissa looked at Ryan.

Then at me.

Her voice barely came out.

“It says…”

She swallowed.

“‘If my son ever has a daughter, you must make sure she never leaves your family.’”

Ryan closed his eyes.

I stared at the baby.

The mark on her back suddenly felt like something much bigger than a birthmark.

Then Melissa turned the page over.

There was another line written on the back.

And this time, even Ryan looked terrified.

Because it wasn’t about the baby.

It was about me.

The final sentence read:

“The woman carrying my granddaughter is not who she thinks she is.”

My hands went numb.

And for the first time since giving birth, I wondered whether my brother had asked me to become his surrogate…
